TB skin test
Baseline blood work (baseline CBC, renal and liver enzymes, HBV)

Ensure all age appropriate immunizations are current.

DOSING

Induction 200 mg IV wk 0, wk 4, wk 8

SC induction 400 mg SC wk 0, wk 4, wk 8 available with approval from medical. Each 400 mg dose is given as two injections of 200 mg

Then maintenance 100mg SC at wk 16 and every 8 wks thereafter

A dose of 200mg SC at week 12 and every 4wks thereafter may be considered for patients who do not show adequate therapeutic benefit or according to clinical judgement.

No dose adjustment is needed

TIME REQUIRED

Minimum 1 hr infusion and < 5 min SC injection

ROUTINE MONITORING

Blood work q 3 months including CBC, liver enzymes, lipids, and renal function

Annual cervical cancer screening – pap test

Annual skin exam – skin malignancies

Influenza vaccine recommended

Screening for osteoporosis with bone mineral density testing periodically after diagnosis
